Summary of Role

 

We are recruiting on behalf of Creative Support for a Mental Health Pathways Coordinator . You will work alongside our Service Manager and Community Links and Assessment Co-ordinator to provide an outcome-focused support service for people over the age of 18 who are identified as being ready to move on from hospital, residential and other supported settings. Priority will be given to people subject to CPA/Section 117 and those who are experiencing delayed discharge.

  

The purpose of this role is to work to source and secure accommodation on behalf of the individuals referred, working with the individual, a range of mental health professionals and a number of housing providers, both social and private to ensure the best outcome for the service users supported. The service is commissioned by Doncaster Borough Council to support people with mental health needs in their journey towards community based housing and independence. 


You will provide advice and guidance as to the range of possible housing and support options including support to locate and resettle into an independent tenancy with support from the Mental Health Floating Support Service, who will then provide a programme of planned support to continue to sustain tenancies.

 

To ensure this is achieved in a timely fashion, you will help to shape pathways between mental health care providers (in-patient and community) local housing providers (both social and private and non-statutory organisations for individuals who have been identified as at a being stage in their recovery where they can sustain an independent tenancy. You will be required to have a good understanding of the local housing provision and work to create capacity for those you are supporting to be housed suitably, in an area where housing options are limited. This will require you to be creative and to create good partnership working with a range of providers.

 

As a Mental Health Pathways Coordinator, your primary responsibility will be to manage referrals specific to the Pathways Service. This includes handling ward referrals and ensuring timely processing to support ward staff in identifying the needs of individuals admitted to various wards in Doncaster, as well as Doncaster residents placed out of area.


Key Responsibilities:

  • Manage and process referrals specific to the Pathways Service in a timely manner.
  • Support ward staff in identifying the needs of individuals admitted to various wards in Doncaster and out-of-area placements for Doncaster residents.
  • Develop and maintain strong links with both social and private landlords.
  • Collaborate closely with the housing network and floating support colleagues to ensure comprehensive support for service users.
  • Playing a crucial role in facilitating seamless transitions for individuals within the mental health system and ensuring they receive the necessary support and accommodation.
  • Working with housing providers to identify suitable move-on options for those in hospital/residential and other supported settings;
  • Creating pathways for a successful and timely move on for inpatients with support from mental health professionals and housing providers;
  • Working to secure properties that meet the needs of the individuals you are supporting, by joint working with local housing providers and mental health professionals;
  • Identifying support options from all available statutory, voluntary and peer-led sources;
  • Advising and supporting individuals in accessing housing and support options which reflect their needs and support their personal journey to recovery;
  • Providing benefit screening and advising of the financial implications of housing options;
  • Building close links/partnerships with all mental health services both locally and relevant out of area services;
  • Developing service level agreements with relevant stakeholders (namely Mental Health and Housing providers) to ensure a smooth and effective move- on pathway for service user;
  • Undertake assessments and establish a move- on plan for those in hospital/residential settings, linked to their statutory care plan;
  • Positive management of risks associated with moving into the community;
  • Ensuring a smooth transition between hospital and home;
  • Handing over of service users into  the floating support service when resettled;
  • Assist the service manager with Case management Stakeholder liaison;
  • Performance management;
  • Reporting against agreed KPIs, outputs, outcomes and contract expectations;
  • Supporting the service manage to ensure quality standards are met;
  • Ensure excellent record keeping;
  • Compliance with all legislative requirements (Health and safety, data protection, etc.).

Experience Required:

  • Ability to assess needs and devise appropriate housing and support plans
  • Experience of carrying out needs/risk assessments with individuals with mental health needs and developing support plans
  • A good understanding of the housing and support needs of individuals with mental health problems
  • A good knowledge of housing options, the housing system and housing pathways
  • Knowledge of the mental health system, legislation (MHA, CPA, MCA, etc) and the roles
  • Experience of providing support to people with mental health needs
  • Ability to undertake basic benefit screening and advice regarding benefits and personal finances
  • A commitment to the Mental Health Recovery Approach and person- centred values and practice
  • Good verbal communication skills and ability to listen sensitively to others
  • Good written communication and report writing skills
  • Proficient in the use of IT and data management
  • Ability to work positively and  flexibly as part of a team
  • A warm, respectful and person centred approach to working with service users and their families, with the ability to provide non-judgemental, respectful advice and support
  • Ability to liaise in a professional manner with other agencies
  • An understanding of the aims and principles of Creative Support

Hours of Work                             

Full time, hours are 37.5 per week. To be worked flexibly on a rota which will include evenings, weekends and public holidays according to the needs of the service.
